Set-up criteria for re-buy tournaments?

I just ran a re-buy tournament, $40 buy-in, 7,000 starting chips, 10 minute blinds, and re-buys allowed within the first 6 levels as long as player has 500 or fewer chips.
I specified:
Add-on chips: 7,000
Re-buy levels: 6
Re-buy threshold: 500
Maximum re-buys: 1

My intention was that players could re-buy 7,000 more chips within the first 6 levels, as long as player has 500 or fewer chips. If player has more than 500 chips, then re-buy is not allowed.

I ran the tournament, and ALL players were given the option to re-buy, at 6 blind-levels at time of the first break -- regardless of how many chips player has. 3 players chose to re-buy, and they all had more than 500 chips.

What gives? What did I do wrong in configurating this tournament?

Please export that tournament to a file and send to me via email. Also include the hand history files that showed the stacks just before and after the rebuys.

I replied via email but will here also. Only 1 player had a rebuy, 3 had add-ons. Add-ons are not subject to the rebuy threshold. Everyone can purchase an add-on, regardless of stack size.
