A player first buy-in (I think), I can ask him. Bought in for $100, and was not in the hand in a standard Hold'em game. His stack showed only $98 while the hand was still going on. I do not have any pulls from the pot or fees of any kind.

Did he post a $2 blind? Show me the hand history in your data folder (System tab -> Help button -> Locate data folder).
