Stop Getting Blinded In + Start Cash Games With All Player

For a simple ring game, it would be great to start the game only when all players are in game or ready. Doesn't seem to be a way to do it unless all players can hit "I'm ready" at the same time.
And for the few that are able to time it right, they get blinded in. This is also a problem when sitting out and having to pay the blind or wait for BB when re-joining. I would like to be able to disable being blinded in.
Hopefully I didn't just miss these settings somewhere.
